Terms of Agreement & Consent

By receiving services at our studio, you acknowledge that you have read, understood, and agreed to the following terms and conditions.

1. Compliance with Louisiana Health Regulations
Our studio strictly follows the Commercial Body Art Regulations established by the Louisiana Department of Health under the State of Louisiana. All procedures are performed in compliance with applicable health, sanitation, and safety standards governing commercial body art.

2. Age Requirement
By signing this agreement, the customer confirms that they are 18 years of age or older, as required under Louisiana law to receive our services.

3. Right to Refuse Service
Our studio reserves the right to refuse service to any individual at our discretion. This may include, but is not limited to, situations involving intoxication, health concerns, inappropriate behavior, inability to provide proper identification, or any circumstance where providing service may be unsafe or unlawful.

4. Sanitation and Equipment
All procedures are performed using single-use, sterile, and disposable equipment where required. All tools, needles, and materials are handled and disposed of according to sanitation standards required by Louisiana commercial body art regulations. Reusable equipment, if applicable, is properly sterilized in accordance with state health guidelines.
 

5. Acknowledgment of Risks
All our services involve inherent risks, including, but not limited to:

• Infection

• Allergic reactions to pigments or materials

• Skin irritation or inflammation

• Dissatisfaction with the final appearance

 

By proceeding with the service, the customer acknowledges these risks and accepts full responsibility for their decision.

6. Health Responsibility
The customer affirms that they have disclosed any known medical conditions, allergies, skin conditions, or medications that may affect the procedure or healing process.

Our studio is not responsible for any health complications or medical conditions that may arise after the procedure, including those related to improper aftercare.

 

7. Aftercare Responsibility
Proper aftercare instructions will be provided. The customer understands that following these instructions is their responsibility. Failure to follow aftercare guidelines may lead to complications, which are outside the control and responsibility of the studio.

8. Legal Responsibility
In the event of any legal dispute, claim, or lawsuit arising from services performed at our studio, the customer agrees that they will be responsible for all costs associated with the legal proceedings, including attorney fees, court fees, and other related expenses.

 

9. Tattoo Design Approval

Prior to the tattoo procedure, the client will have the opportunity to review and approve the final tattoo design, size, placement, and orientation on the body.

By proceeding with the tattoo procedure, the client confirms that they have carefully reviewed and approved the design and placement. Once the tattooing process has begun, the design and placement cannot be changed or reversed.

The client acknowledges that tattooing is an artistic process and that slight variations in color, shading, and line work may occur due to skin type, healing process, and other factors beyond the control of the artist.

The studio and artist are not responsible for dissatisfaction related to design choices, placement, or aesthetic preferences that were approved by the client prior to the procedure.

10. Consent
By submitting the Service Request Form, the customer confirms that:

• They are at least 18 years old.

• They have voluntarily chosen to receive our service(s).

• They understand the risks associated.

• They agree to all terms listed in this agreement.
